It's time, baby. We're closing out season 1 cuz season 2 is on the horizon. The pit episode 15 at your request. Huge thanks to Zach for sponsoring this video. Let's get started. Boop.

We, have, had, 112, mass, casualty, patients come through here in the last 3 hours. Only six people that we couldn't save. They're still in our makeshift Morgan Pedes. 71 people went upstairs. Last I heard, surgery had stabilized the 39 most critical red zone, 32 pink zone patients. We also had 24 yellow zone patients.

The, fact, that, he, can, just, rattle, this off, not as an actor, as a doctor is impressive. And they can do this. This is what happens when you get ultra focused, ultra zoned in during your job. It's entering the state of flow.

The, police, are, still, interviewing. Everybody else is still down here. We discharged eight people. And I don't know the status of the green zone patients because I wasn't there. You'll have to check with family medicine.

That's, me. You ready to give this measles kid to Dr. Shann and go home? Oh,, no., I'm, I'm, okay.

Are, you, sure? Yeah. It's just you, know,, we've, repeatedly, tried, to explain the importance and low risk of the spinal tapment. And the parents are

they're, risking, their, son's, life. Hopefully,, they'll, come, around. And if they don't

we, do, the, best, that, we, can., Some, things are out of our control. This is uh something I'm a bit surprised by because if parents are putting their child at obvious risk, I think uh consultation with a bioethicist or someone in the hospital, the legal department uh needs to be had because while parents do have final say over their children's bodies to some degree, if you're doing obviously harm to your child, that also constitutes as negligence. So, this needs to be figured out and fleshed out a bit further. And it's sad that we're even talking about measles. Uh unfortunately, the reality is we're going to be talking about measles a lot more in the United States given the fact that we have a true antivaxer as the head of HHS. Measles cases are spiking.

Here are the numbers from the last 20 years. And as you can see where we are today, thanks to Secretary Kennedy because he thinks natural immunity is so much better than vaccine immunity. We're going to have a lot of measles and where it's most problematic is in those who cannot yet get a measles vaccine because they are too young in addition to obviously all the other children. Initially this was more of an issue in isolated pockets where there were large unvaccinated groups but now it's spreading. It's here in the northeast.

There's local uh hospitals that have put out bulletins that they have patients with measles. Sad.

Dr., Shen,, how, are, we, progressing, in here? No improvement in the patient or his family.

Where, is, your, wife, and, daughter,, Mr. Edwards? My wife took Georgia to the bathroom.

Okay., Well,, your, son, Flynn, is, critically ill, and the longer that we wait, the worse this could get. Without the spinal tap, we don't know the best treatment to give him to give him the best chance of recovery.

What's, going, on? We, were, discussing, the, options, for treating your son. Diagnosing.

He, already, looks, better. How, is, she?, Ask, her, what, is, giving, you that impression because you as a physician don't see that improvement.

He, is, not., And, by, refusing, the, spinal tap, we don't know how to help him. The steroids could kill him if it's just pneumonia. The steroids could save him if it's a himid. And the longer that we wait, the greater the risk of him dying.

Luke, might, die. No,, honey., He's, just, got, a, worse, version of what you had.

I'll, keep, him, intubated,, hydrated,, and pumping the antibiotics. I'll let you know if anything changes.

Mr., was, May, I, speak, with, you, outside, please? This, is, a, very, challenging, situation, especially given the day that he's had. I mean, I don't know what I would do in a scenario like this because families in in this way exist, but you have to reiterate over and over and over again again, from a place of empathy, but at the same time voicing your concern for their child.

Tell, me, about, your, son. She's, a, great, kid., Kind., He's, very, good to his little sister. you too close.

Yeah. I'm, curious, what, his, strategy, here, is. Getting to know a family is obviously wise. Trying to find some point of agreement is valuable. Curiosity about the family's values also go a long way. But I'm curious where he's taking this.

What, the, hell, is, this?, These, are, the people from the Pitfest shooting that we couldn't save today. We couldn't save them, but we can still save your son. You're an

Yeah,, this, is, a, bit, dramatic., If, these were children that died of measles maybe you could make an impact, but it's just so unrelated. It feels not genuine and inflammatory.

It's, a, code,, Tam., Illness, or, injury, in, a public area of the hospital, usually have fainting or fall.

Yeah., Every, hospital, has, different colors for their codes. It's usually written on the back of their badges in order for new employees or if you've forgotten to check what each code means. We did have a similar code like that that was specifically different than rapid response because a rapid response was for a patient that was admitted versus a visitor, a loved one, someone that's already going home is a different type of page.

Uh, repeat, labs, back, on, our, blue, man, max running through agents that cause met hemoglobinia, mainly drugs and toxins. Uh, what drugs cause methlobal anemia? Oh, how about methylene blue due to the fact that a lot of people are excited about it? Live fact check. Oh, methylene blue is the antidote for meth hemoglobinia. I believe spraying throat numbing spray, overusing it which we actually have a video talking about the dangers of that, I believe also causes that.

Yeah., Drugs, like, topline, aesthetics, FROM THE DENTIST. OH, LET'S GO. LOOK at that memory functioning.

Also, ask, about, industrial, toxins, analene dyes, benzene. There's also food additives nitrates nitrites.

Some, of, those, are, available, online, as kits. Think, he, may, have, tried, to, hurt, himself. Here, it's important to ask the question. Uh, and med students residents sometimes are nervous to ask the question for the fear of implanting the thought in one's mind. But actually asking the question outright yields the best outcomes. I

mean,, if, this, was, a, failed, attempt,, it makes sense that he's too frightened or embarrassed to admit to what he did and too afraid to go home because of what he might still do.

Here's, a, great, way, to, simplify, this scenario and what I would tell my resident. Did you ask the patient if they had any SI or hi? No. Well, go ask him and let's find out what he says. Maybe then you'll get your answer. Okay I'll call Psych, but find someone to take over for you. Your shift ended a couple of hours ago.

I'd, like, to, try, and, get, a, little, more out of him. See if I can get him to confess to something.

So, then, maybe, you, can, call, the, consult for psychiatry and the crisis team and ask what questions you should ask. It's never bad to call a consult and then at the same time ask for advice on what you can do in the meantime.

Pelvic, crush, injury. Don't, even, think, about, it., Thought, we were close to trauma.

This, is, the, code, tan., He, got, pinned behind a truck backing up with replacement supplies.

Oh,, pelvic, fractures., Not, only, very painful, but very dangerous. Lots of vital structures there, including large arteries that can cause a lot of bleeding. You know, we've talked about femur fractures leading to fatty embolis, which can spread quite quickly and into the circulation leading to death.

Oh, my, god. Okay,, I, got, the, efast., Grab, a, binder. Obvious pelvic fracture.

I'm, in, a, lot, of, pain., you, taking, any medications pressure? Any, drug, allergies?

They're, basically, doing, a, scan, to, figure out whether or not there's bleeding internally. Uh so they can check different sections of the abdomen and see if blood starts pooling.

Whole, blood, massive, transfusion protocol. Uh, we, got, whole, blood, coming, in, from here in Youngsttown. I'm not sure it's here yet.

Let's, go, one, to, one, to, one, red, cells, platelets, and plasma. We've got that. Let's place an IG after the intubation please.

On, it. They're, trying, to, make, sure, that, he doesn't have tremendous amount of blood loss. that if he does, they're uh working to replace it. Most importantly they're trying to get good access. So putting in a line up here uh will give them better access uh faster ability to replete any blood losses he might have.

Central, line, is, in. Let's, hook, up, the, rapid, diffuser, over, to the J and then we can shoot the film. Clear for X-ray.

That's, a, portable, X-ray., At, times, of trauma like this, it's very valuable to figure out uh a diagnostic approach. Image quality is not always as good as obviously a true X-ray where you can get multiple views.

Oh,, that, ain't, good. Does, he, have, crushed, pelvis, as, imagined? Huh?, Wide, symphysis, pubis, anteriorly,

disrupted, sacroiliac, posteriorly., Guy's bleeding like a stuck pig. I, got, Dr., Walsh, on, speakerphone, from, the O.

Hey, guys,, what's, up? We've, got, an, unstable, pelvic, rig fracture systolic of 68. The fact that his systolic is 68 after they're giving them all these bololises of fluids blood, that's a really bad prognostic sign. Back to the show in just a second but first I want to tell you about my sponsor, Zachdoc, a free website where you can easily search and compare highquality in-et network doctors so you can find the right doctor for you. Look you already know how big of a headache it is booking appointments or finding a new doctor. But you might be surprised to learn that doctors themselves struggle with this, too. Yeah, just because I get to wear scrubs and a white coat to work every day doesn't mean I have the perfect understanding of every hospital and doctor's booking procedure.

Recently, my father, who is also a family medicine doc, had a personal health problem of his own and needed to see a new doctor. We ran into the same problems you do. Confusing websites endless voicemails that truly never get returned, you name it. So, we turned to Zachdoc to find the doctor he needed. And boom, we found that perfect person. They specialized in exactly what my dad needed, were within a reasonable distance of his home and actually had openings when he was available. My dad's the type of person who could have just gotten so frustrated with the entire thing and given up entirely. We need Papa Arshopski in good shape, guys. I can't have him ignoring his own health.

My favorite part that you can see doctor's appointments openings, choose a time that works for you, and click to instantly book a visit. Zakdoc appointments also happen so fast you can even score same day appointments. You don't even have to talk to anyone. Just book with a few clicks. So stop putting off those doctor's appointments. Go to zakdoc.com/drmike, also linked in the description to find and instantly book a toprated doctor today.

Yes., Yes. How's, he, doing? Same. So, he's, not, getting, any, worse? Uh, not, at, the, moment., Our, family physician is making arrangements to move him to West Penn.

What's, going, to, happen, in, West, Penn? That would be my question. What is Westpen hoping to do or ask to speak to the physician? This is a valuable time to be inquisitive.

Will, you, excuse, me, for, a, second? After, three, rounds,, pack, cells,, FFP,, and platelets. That that was a good scene showing how the hospital uh really has different things going on at the same time where in one place it seems really quiet and calm and you just open one door and you're just in a whole different reality.

Thanks, for, letting, me, come, back. I, didn't, let, you, let, you, come, back.

I, knew, he, was, going, to, say, it., It, was, so It was the layup. I, I, was, never, high., I, was, just, treating my own withdrawal symptoms.

Well,, you, should, have, done, that, under the care and supervision of a physician. With that on my record, I could I could I could have my hospital privileges denied whenever I apply for a job.

No., No,, that's, not, true, anymore., That's not true anymore. You could have come to me.

Instead,, you, could, be, arrested, on, felony charges for stealing controlled substances from the hospital. Robbie I could lose my medical license.

Why, didn't, you, think, of, that, yourself? Come, on,, man., You, never, heard, of, second chances. Here's your second chance. 30-day inpatient treatment program followed by random urine tests 50 to 60 a year followed by mandatory NA meetings three to four times a week for the first three years

of, a, five-year, program., You, need, help. What, about, you,, man? What, about, you?, I'm, not, the, only, one who's a little up here, Robbie. Why don't you look in the mirror?

What's, that, supposed, to, mean? I, never, had, a, complete, meltdown. No,, you, just, caused, them, in, other people.

You're, blaming, me, for, what, happened, to you. What, happened, to, me? What happened to me? Yeah you are so full of You let me down. You let everybody down, especially yourself. Someone saw you in peace.

Oh, my, god., He, had, an, emotional, moment. That's not the same as your This is gaslighting at its finest.

Systolic's, down, to, 64, to, 38., He's, tagged to 132. Pelvic fracture. Yeah., Rechecking, for, delayed intraabdominal bleed.

Thought, you, said, he, was, stable. He, was, after, three, units., and, he continued to improve. Pressure got up to 118 systolic after the full MTP

systolic, of, 118. It, was, 90, after, three, units. He, gave, him, too, much, blood, and, raised his pressure so high that it blew apart the developing clots that was stopping him from bleeding out.

First, round, of, MTP, is, six, units, red cells, six FFP, six platelets. That's, major, trauma, protocol.

Treat, the, patient,, not, the, protocol. Treat, the, patient,, not, the, lab., But, in this scenario, tough to make that argument.

I'm, sorry, if, I, gave, him, too, much, blood. Dr. Alice, now that Dr. Shen is here, we have plenty of hands on deck. Why don't you go check on your other patients?

Hey,, what, about, Roboa?, The, balloon, cuts off blood to the lower half of the body.

With, bilateral, crush, injuries,, we, don't know if we have an intact artery to thread it in.

You, could, always, crack, the, chest,, cross clamp the aorta, and stabilize. These, are, very, aggressive,, very, unique measures.

I, got, a, better, idea. Yeah. Prepariteneal, packing., Simple, midline incision below the umbilicus. Pack like hell around and behind the bladder. That pressure should stop the bleeding until we get him upstairs. Again, the the purpose of doing all these procedures and discussing these procedures is to clamp off bleeding, in order to stop the bleeding, in order to allow the surgeons to get in there and figure out what tissue can be saved. And there's obviously different approaches depending on where the injuries lie. given the fact that they don't know where the injuries lie makes the situation that much more tricky and therefore they're doing this experimental combat level approach that obviously I'm not super familiar with but they've done them at least several times so they do have some experience

Dr., King,, we, want, you, to, do, the, spinal tap before Flynn gets any worse.

Great., I, will, meet, you, in, there. What, changed?, I, I What happened? I, had, a, a, good, friend who um went through some really ugly stuff.

Look,, having, these, types, of, emotional conversations and creating a bond could be valuable and it's probably good for the show, so I'm not going to hate on it too much. But in the huge majority of times, it doesn't need to be this dramatic. It it could be as simple as connecting, putting your arm on someone's shoulder, telling them that you're there for their best interest and you're really trying to figure out what's going on, and you ask them some direct questions. And usually you get the answers. And if you don't, you at least get some hints towards those answers. Curious where this will go. And things got so bad that my friend there isn't a day that I don't regret not being able to stop my friend.

The, reason, why, I, don't, like, these stories without a full understanding of the picture is I'm not sure that this story is beneficial for the patient or beneficial for the doctor. And if you're doing something that's good for you but not the patient, that's already uh a wrong issue. And then you don't know if by presenting the story in this obscure way can upset the patient, can lead them to have worse feelings about their issue, can lead them to feel like you're judging them. So being straightforward is always the best approach because then no one will try and find ulterior motives and if they do, you can explain them away because you're just being straightforward in order to make the best diagnosis and to help the patient.

I, just, don't, want, to, be, here, anymore. asking some more questions, continuing that conversation. Physical support is okay, but just being very dramatic like that isn't always the optimal approach. At least, not, in, my, mind.

Here, come, more, lads. Dropped, them, on, the, mail. Templated, to, me. I've, done, a, hundred, of, these.

Well,, that's, good, that, he, has, that, much experience, Matt. Okay. You're also cutting pretty superficially to cut the bladder.

Once, I'm, through, the, fascia,, you're going to sweep a finger around the bladder. Open up the space around us. All right., Three, lap, pads, on, each, side of the bladder deep as you can.

How, interesting, is, this?, The, big, idea, the big rescue is just jamming in pads. But that that can work.

One, more, check, of, the, transverse, view perfectly in the midline. The longitudinal view. See, in a family that was so nervous about doing the spinal tap, allowing this to be the one where you teach a resident how to do it is not the best approach, just putting it out there. At least ask the family about their choice in that regard or give them a choice. Just imagine if something even mild goes wrong. All the blame is going to be on the fact that this is a young inexperienced doctor. Think about the liabilities, the disbelief that comes with it.

Okay,, Dad,, we're, in., It's, going, to, take a minute to collect but, the, fluid, looks, clear.

That's, good,, right? It's, a, good, sign., The, lab, still, needs, to run some tests, look under the microscope to rule out some bad stuff.

Based, on, uh, the, consistency,, the, color of uh what comes out here, you're going to get already a better understanding of what potentially could be causing these symptoms.

What, are, you, doing? You, said, she, didn't, want, to, be, in, the room. I, was, getting, Georgia, something, to, eat. Wait, wait outside.

Stop, what, you're, doing, right, now. No,, they're, almost, done. I, can't, believe, you, went, behind, my, back.

I, did, what, was, best, for, Flynn,, not, for you. Take, that, needle, out, of, his, back.

No. Yes, ma'am., I, don't, want, to, have, to, ask, you to leave. Take, it, out, right, now.

Yep., Coming, out. All done. My son is leaving this hospital. Documenting, here, is, going, to, be, so important. Everything that happened needs to be documented in the chart.

Fifth, lap, pad, in, place. Yep. I, believe, they, were, going, for, six, total.

Number, six., And, we're, done. Okay. BP92, over, 48. Hey,, I'll, stay, with, him, till, I, ready.

Okay., By, yourself? Yeah., needs, to, make, sure, he, doesn't, get another drop of blood.

Hey,, you, okay? Yeah., I, mean,, it, there there's, a, thing with these shows where it seems like the characters are disconnected from one another on just a completely different level. There was a mass casualty event today. If I I'm just putting myself in that position. If I see one of my colleagues that I just came on to on shift and I see one of them is a little bit out of it, I'm not going to wonder like what's going on with them, I'm gonna completely understand that they just went through a massively traumatic experience.

We, saw, 112, mass, casualty, patients, come through here in the last 4 hours and 106 of them are going to live. None of us are going to forget today. even if we really really want to. So go home, let yourselves cry

although, not, mandatory, you'll, feel, better., It's, just, grief. Leave me the body.

Everyone, grieavves, in, their, own, way., I believe is true. And the human mind has a tremendous uh ability to bounce back from these types of traumatic events. It's important to know that everyone has a slight not everyone but a lot of people have different timelines for which they bounce back. This there's been some uh great work done by a psychologist by the name of George Banano who we had on the podcast discussed this at length.

That's, the, interesting, thing, about, the ER. It never stops. Like you just had an incredible shift and then it just it keeps on going. And imagine now because our government is anti-giving people vaccines, we're gonna start seeing more of these acute cases rolling in. And for what? We're cutting down research dollars from vaccines that can potentially prevent cancers. Like not sick of working nights yet?

My, therapist, thinks, I, find, comfort, in the darkness. That, sounds, like, a, Spider-Man, quote. give you his number if you ever feel like talking to him.

Does, it, help? I haven't jumped off the roof, have I? It, does, help., And, the, reason, that, it does help is a lot of times we have emotions and feelings and thoughts that are coming from basically our lizard brain, our brain that is quick to feel uh highs, lows, catastrophizing, but not really thinking things through. And by journaling, by doing therapy, we actually move these thoughts into the more thoughtful parts of our brains where we can develop rational feedback for those thoughts, for those feelings and therefore potentially give ourselves a higher level of control. Again therapy, uh, mental health medications are not about making you into a robot into never feeling sad, anxious again.

It's about giving you back that control. Actually, sorry. I don't drink. I don't know why I took that. She's not old enough.

I'd, say, if, she's, old, enough, to, put, it, in a chest tube and intubate, she's old enough to drink a beer.

Ah,, that's, illegal., Can, you, believe, that on my first day of medical school, I wasn't allowed to go to the opening party cuz I wasn't yet 21. I did one of those accelerated programs. So, I couldn't go. I said I have a fake ID though.

Guys,, do, us, every, not, always., Usually, it's a little more lively. Or the emergency department throws wicked parties.

What's, so, funny? I, just, realized, this, is, your, first shift. I, will, drink, to, that.

I, hope, none, of, them, are, driving, home, cuz that would make for a bad second season start.

Okay,, that's, it, for, me., Beer, for, the road. Yeah,, why, not? Who brings this cooler full of beer to the hospital?

Good, night. Get some rest. Tomorrow's, another, day. ER, doctors, also, and, ER, staff traditionally don't necessarily work every single day. They have a very unique schedule uh depending on obviously where they work and which uh departments in which they work, but it's usually not every day. And the reason they don't work every day is cuz shifts are like this a lot of the time.

I can't wait for season two. We need season two now. I love how Dr. Robbie tries to speak with people who disagree with him. I actually have a whole video about that from my latest Jubilee debate. As always, stay happy and healthy and check it out.
